Paperwork - Due November 1st

All Athletes and Families require the following paperwork. Please read all instructions carefully so that your paperwork is accepted. PRO TIP: Most of these are easiest to complete on a computer vs. your phone.

1. Player & Family Handbook | ALL TEAMS

3. Uniform Fitting & Order | ALL TEAMS

4. Lindsay’s Law form | ALL TEAMS

5. Ohio Concussion Awareness Law form | ALL TEAMS

6. JVA Release of Liability form | ALL TEAMS

  • Choose Mintonette Sports as your club and use the following:
  • Club Director Name: Katie Butz
  • Club Director's Email: katie@mintonettesports.com

7. Scan Birth Certificate | ONLY NATIONAL TEAMS

  • If new to the Club or it is your first time on a National team, please scan and email a copy of your birth certificate to katie@mintonettesports.com.
  • You must send a PDF copy. We cannot accept a picture of the certificate.
  • American and Regional teams DO NOT need to send these.

8. AAU Membership | ALL TEAMS 

  • CLICK HERE first to create your NEW PARENT ACCOUNT in the new AAU Uventex system. Click the "Sign in" button, then the "Sign up" link.
  • CLICK HERE to purchase your AAU membership. Be sure to select Mintonette Sports as your club using our NEW 2025-26 club code: UFQFDV7HW. 
    • Note: the system will give you an option to verify their age. You can skip this and "Check out without age verification".

9.  USAV & OVR Membership | ONLY NATIONAL & REGIONAL TEAMS
 
(not required for American teams - m.65, m.55, m.56, m.45, m.46, m.34, m.35, m.23)

  • You should already have a tryout membership for team tryouts, but this step is upgrading to the full-season membership. 

    • To upgrade to a full membership/add your membership to our club, you must use our custom Mintonette link, not the generic OVR membership link.
    • Please be sure to choose your athlete when it asks, "Who's this Membership for?".
    • When asked about the primary role, please choose "Player/Athlete".
    • When asking how long, choose "season".
    • It should take you to a screen that says "25-26 OVR Junior Player", select "next"... follow the prompts.
    • When it comes to the payment page, you will see two charges: $48 for the OVR and $24 for the USAV. This is correct.
    • After you pay, it will say "Almost Done!".  Click to go to your account.
    • You will need to make sure that it says "Eligible" & "Mintonette Sports" under your athlete's profile for the 25-26 OVR Junior Player membership.
    • If it does not, then click to view more details of what needs to be completed.

    **If you already have a membership, you will end up skipping some of the steps above**

10. OVR Junior Officiating Course | ALL TEAMS

  • Please use this resource for your officiating responsibilities. You can access the online modules to learn how to officiate. These must be done prior to your first event.
